The Menopause Moment: Why Everything You Think You Know About Hormones Is Wrong

Let me ask you something—when did we decide that women should just "suffer through" menopause while men get Viagra and testosterone like candy? When did we collectively agree that hot flashes, brain fog, and feeling like you're losing your damn mind was just "part of being a woman"?

I recently sat down with Dr. Kelly Casperson, one of only 1,000 female urologists in the United States and author of "The Menopause Moment," and what she told me made my blood boil—in the best possible way. This conversation isn't just about menopause symptoms; it's about how the medical establishment has systematically failed women for over two decades, and what we can do about it right now.

The Study That Broke Everything

Here's what happened: In 2002, the Women's Health Initiative study looked at whether putting older women (average age 63) on synthetic oral hormones would decrease their risk of heart disease. The answer was no. But here's where it gets infuriating—the media took the nuanced results and screamed "HORMONES CAUSE BREAST CANCER" from every rooftop.

Dr. Kelly breaks it down: "They went to the media, the media took it and ran. The media said hormones cause breast cancer and the rest is history." What followed was a two-decade moratorium on hormone education. Doctors stopped learning about it. Women started suffering unnecessarily. And here we are, with 80 million women over 40 in this country and a massive gap in care.

The "Too Young, Too Old" Bullshit

Want to know something that'll make you see red? Men never get told they're "too young" or "too old" for hormone therapy. Never. But women? We get gatekept at every turn. "Oh, you're 42? Too young for hormones." "You're 65? Too old now, should have started earlier."

As Dr. Kelly puts it: "There's nothing in medicine that says age is an absolute iron curtain on having something or not having something." We have ages for voting, driving, buying alcohol—but medicine doesn't work that way. Risks go up with age, sure, but that doesn't mean treatment should be off the table.

The Pellet Problem

Let's talk about hormone pellets because everyone's asking about them. Yes, they work for some people. Yes, they can eliminate hot flashes. But here's what the pellet clinics won't tell you: they're the most expensive option, they're not FDA approved for women, they're invasive (hello, incisions), and if the dose is too high, you're stuck with it for three months.

Dr. Kelly sees a lot of complaints about hair loss with pellets, and here's why: "Hair follicles hate sudden changes—postpartum, thyroid, sudden weight loss, stress, massive hormone changes, hair loss." When you go from nothing to really high hormone levels overnight, your hair freaks out.

Your Lab Results Aren't Your Identity

Can we please stop with the "estrogen dominance" labels? I was guilty of this myself—got tested, was told I was estrogen dominant, and held onto that like it was my astrological sign. But Dr. Kelly set me straight: "That was on the day of your test. Don't not be telling me when you're 72 that you're estrogen dominant. It is not a fixed trait."

Perimenopause means your hormones are fluctuating wildly. What you test today won't be what you test next month. The goal is to treat your symptoms, not chase numbers on a lab report.

The Brain Connection You Need to Know

Here's something that blew my mind: estrogen, testosterone, and progesterone are all neuro-hormones that help your neurons function. When estrogen drops, your brain literally can't process glucose as well. That brain fog you're experiencing? That's not early dementia—that's your brain missing the hormones it needs to function optimally.

And here's another mind-blower: quadriceps strength is directly linked to dementia prevention. Dr. Kelly explains: "Muscle is literally brain health. You tell your brain that you need muscles to keep going, the brain's like, all right."

The Estrogen Truth That Changes Everything

Ready for this? If there was a drug that could decrease your risk of death and help you live about two years longer, wouldn't everyone be on it? Well, that drug exists, and it's called estrogen. Studies show it can decrease your risk of colon cancer, breast cancer, and heart disease when started at the right time.

But we've been so scared by that 2002 study that we're letting women suffer unnecessarily. Dr. Kelly's fire comes through when she talks about this: "The amount of suffering out there is profound. And what pisses me off is that women are so satisfied with fear being how they want to live."

Taking Your Power Back

So what do we do with all this information? First, we get educated. We stop accepting "it's natural" as a reason to suffer. (Cancer is natural too, but we treat that.) We find menopause-certified providers who actually know what they're talking about. We advocate for ourselves in the doctor's office.

Dr. Kelly recommends checking out online clinics like MIDI Health, Evernow, Alloy, and GenEv—places where treating perimenopause and menopause is literally their job, not something they're guessing at.

Most importantly, we start treating our bodies like the incredible machines they are. We prioritize sleep, exercise, and proper nutrition. We build muscle because muscle is brain health. We stop drinking alcohol because it's literally poison for our brains and hearts.

The Bottom Line

Your midlife transition doesn't have to be hell. You don't have to suffer through hot flashes, brain fog, and feeling like you're losing yourself. There are options, there are treatments, and there are providers who will listen to you.
